In order to complete the verification process for your account, you must submit a proof of address document whose name and address match the name and address listed in your AlertPay account. In the event that you do not have a proof of address issued in your name, we will be willing to accept a proof of address document where the last name and address on the document match that of your AlertPay profile. Your proof of address can be submitted in the form of a phone bill, a utility bill, a bank statement, or a credit card statement. Your proof of address must also be a recurring monthly statement that is no more than six (6) months old.
Your proof of address document must meet the conditions stated above in order to be accepted as vaild for the AlertPay verification process.
